AMPK: the low-fuel sensor that exercise and metformin both switch on

AMPK is the yin to mTOR's yang — the signal that says the cell is short of energy and should start burning fat, building mitochondria and tidying up. Most "longevity" interventions pass through it.

What it means

AMP-activated protein kinase is an enzyme that activates when the ratio of AMP to ATP rises — in other words, when the cell is using energy faster than it is making it. Once active, it switches on glucose uptake, fatty-acid oxidation and mitochondrial biogenesis (via PGC-1α), and switches off energy-expensive building processes, including mTOR.

Exercise is the strongest physiological activator. Metformin activates it pharmacologically. Calorie restriction, cold, and several plant polyphenols (resveratrol, berberine) activate it to varying and often over-stated degrees.

Why it matters for longevity

AMPK activity declines with age, and falling AMPK is linked to the loss of mitochondrial quality, insulin sensitivity and autophagy that characterise ageing tissue. In animals, activating AMPK extends lifespan; in humans, the interventions that activate it — training and weight loss — are the ones with the best outcome data, which is suggestive rather than proof.

What the evidence says

Genetic activation of AMPK extends lifespan in worms and flies; metformin, which works partly through AMPK, extended lifespan in male mice in some ITP arms and not others. In human muscle biopsies, a single bout of exercise raises AMPK activity within minutes, and twelve weeks of interval training restores much of the age-related loss of mitochondrial function (Robinson 2017).

The supplement claims are weaker. Resveratrol activates AMPK in cells at concentrations the blood never reaches orally; berberine activates it and lowers glucose in trials but has gut side effects and drug interactions. Nothing taken as a pill has matched what training does to AMPK in a person.

Does metformin blunt exercise gains through AMPK?

The 2019 MASTERS trial found that metformin reduced the VO2max and muscle gains from training in older adults. The effect is real but modest, and dose-timing strategies are being studied.

Is berberine "natural metformin"?

It activates AMPK and lowers glucose in trials, but it is not metformin: bioavailability is poor, gut effects are common and it interacts with many drugs.

Can I activate AMPK with cold exposure?

Cold raises AMPK in brown fat and muscle in rodents and acutely in humans. Whether that adds anything to training is unknown.
