What it means
Sex hormone-binding globulin is a glycoprotein made by the liver that binds testosterone and estradiol tightly in the blood. Hormone bound to SHBG is inactive; only the free fraction (about 2% of testosterone) and the loosely albumin-bound fraction can enter cells. SHBG therefore sets the free hormone level for any given total.
It rises with age, oestrogen, thyroid hormone and low insulin; it falls with insulin resistance, obesity, androgens and high insulin — the liver makes less SHBG when insulin is high.
Why it matters for longevity
Low SHBG is one of the earliest markers of insulin resistance and predicts type 2 diabetes independently of glucose. High SHBG in older men lowers free testosterone and produces deficiency symptoms with a "normal" total — the pattern that gets missed. Reading SHBG with total testosterone is what turns a hormone panel from a number into a diagnosis.
What the evidence says
Ding 2009 (NEJM, Women's Health Study and Physicians' Health Study): the highest quartile of SHBG had about 80–90% lower risk of type 2 diabetes than the lowest, and genetic variants raising SHBG lowered diabetes risk — evidence that the relationship is causal, not just a marker. Low SHBG also predicts fatty liver and metabolic syndrome in men and women.
In the European Male Ageing Study, free testosterone calculated from SHBG classified hypogonadism more accurately than total testosterone, especially in men over 60 and in obese men. SHBG rises about 1–2% per year after 40.
How to measure or use it
- A standard blood test, drawn with the morning testosterone; reference range roughly 20–60 nmol/L in men, higher in women.
- Use it to calculate free testosterone (Vermeulen equation; most labs will do it) rather than measuring free testosterone directly, which is unreliable.
- Low SHBG (under 20) is a prompt to check HOMA-IR and liver fat; high SHBG (over 60) with low-normal total testosterone is a prompt to look at the free level before calling the result normal.
